The quest requirements for the Berserker curse were missing from the in-game skill guide. Evil Otto was named after Dave Otto, security chief at Dave Nutting Associates (McNeil's former employer).
The only myth is that two people died while playing the game it was just one and the second kill was initiated at that cabinet then finished in the alleyway.
